TRANSCRIPT REQUEST FORM

To receive a copy of your high school transcript or have it mailed to a college or university, you will need to click  the link below and complete the Transcript Request Form.  Once the form has been completed, you may do one of the following:

1.  Mail the form to:

        New Philadelphia High School Guidance Office
        343 Ray Avenue, NW
        New Philadelphia, OH  44663

2.  Fax the form to the Guidance Office at 330-364-0741

3.  Submit the form in person to the Guidance Office
