REGISTRATION PROCEDURES FOR STUDENTS IN GRADES 1-12

Registration Information necessary to register your child in the New Philadelphia School District:

"PROCEDIMIENTOS DE INSCRIPCIÓN PARA LOS ESTUDIANTES DE GRADOS 1-12 Registro necesario para matricular a su hijo en el Nuevo Distrito Escolar de Filadelfia de la información:           1. Completado los documentos de registro (a menos que hayan sido presentadas en línea a través del sitio web) 2. Certificado de nacimiento original (Debe tener el sello del Departamento de Salud)         3. Tarjeta de Seguro Social o número de            4. Registro de Vacunas          5. Prueba de residencia (factura de servicios públicos que muestre su residencia)           6. Documentos legales relacionados con los derechos de custodia (en casos de divorcio, adopción, etc)

            2.  Original Birth Certificate (Must have seal from the Health Department)
            3.  Child’s Social Security Card or Number
            4.  Immunization Records 
            5.  Proof of residence (utility bill showing your residence)
            6.  Legal Documents pertaining to custodial rights (in cases of divorce, adoption, etc.)

           



    

NEW !!!      KINDERGARTEN REGISTRATION ONLY  - ON LINE FORMS 

          Students who are 5 years of age on or by August 1, 2012 are eligible to attend Kindergarten.  Please access the website to obtain and complete the registration forms. 

Please complete the on-line District Enrollment Packet.

Please bring your child and the following paperwork to your scheduled screening appointment at The Quaker Dome.
            1.  Birth Certificate 
            2.  Child’s Social Security Card or Number
            3.  Immunization Records  
            4.  Proof of residence (utility bill or rental agreement)  Your proof must include your name and address.
            5.  Legal Documents, if applicable, pertaining to custodial rights (in cases of divorce, adoption, etc.)

Please understand that if you do not bring the above requested information, your child will not be on a class list until we receive this information.  The Physician and Dentist report forms are not required to be turned in until the first day of school.

The following directions are from the Court House, 125 E. High Ave., New Philadelphia
1. Head east on E High Ave toward 1st Dr. SE  .. go 2.9 miles - takes about 5 minutes
2. Turn left onto 24th Dr. NE (which is the 1st left past Arby's Restaurant)
